The home page will always let you know whether your most up-to-date health status has been sent to your care team. In case your latest health status has not been sent, you may use the “Answer Your Questions Now” button to provide an update to your care team.
Additionally, the home page provides access to the following features:
- Send a message to your care team, in case you have a health-related question or would like to be contacted by someone from your doctor’s office.
- View your answer history, where you can access all of the health status information you’ve submitted using ChronicCareIQ
- Submit additional health status updates, if your monitoring program supports it
- View or download your care plan (CCM patients)
